Our New Patient Appointments include a comprehensive exam, oral cancer screening, appropriate x-rays, a cleaning (if appropriate) based on condition and health of the gum tissue, and treatment planning catered to your dental needs and concerns.

We ask that you complete this form and bring it with you to your first visit to Riverside Dental. If while filling out the form, you have questions, please do not hesitate to call our office and we will be happy to assist you. If you would like to include other family members as new patients, please print out and fill in forms for each individual family member.
